After Extraction of Wisdom Teeth

In most cases, the removal of wisdom teeth is performed under local anesthesia, laughing gas (nitrous oxide/oxygen analgesia), or general anesthesia. These options, as well as the surgical risks (i.e., sensory nerve damage, sinus complications), will be discussed with you before the procedure is performed. Once the teeth are removed, the gum is sutured. To help control bleeding bite down on the gauze placed in your mouth. Upon discharge your postoperative kit will include postoperative instructions, a prescription for pain medication, antibiotics, and a follow-up appointment in one week for suture removal. What Will I Feel Like after Wisdom Teeth Removal Surgery?

On the first day after surgery, you may experience some minor bleeding and pain. You should cover your pillowcase with something so that you don’t get any blood on it. Each individual’s reaction to surgery varies, and the sensation of pain can range from mild discomfort to severe pain. A variable amount of swelling can be expected following the surgery. This swelling usually peaks on the second day and should begin resolving on the third day. You can limit the amount of swelling you will have by using ice for the entire first day. The more ice you use the first day, the less swelling you are likely to have on the second day. Please remember to put ice on the first day even if it is somewhat uncomfortable to have the cold next to your skin. On the third day, you will notice that your jaw muscles are stiff, and it is difficult to open your mouth normally. You can apply moist heat to your face on the second and third day allowing your muscles to relax more and open wider. Most of the time you will want to limit your activities for a few days. We ask that you follow your post-operative instructions closely. Doing so will make you as comfortable as possible during the first few days following your procedure. Please allow time for your body to begin healing before resuming an active social, academic, or athletic schedule. Most patients feel like they are over the hump and on their way to recovery in 3 to 5 days.

Are There Any Problems after the Extraction of Wisdom Teeth?

As with any medical procedure, there can be complications or an unanticipated result. Some complications that patients undergoing Wisdom Tooth Extraction may experience include: Damage to the sensory nerve that supplies sensation to the lips and tongue, sinus communication, infections and dry sockets.

After the procedure, our assistants will review your post-operative instructions with your escort. We ask that you follow these instructions closely, as they will make you most comfortable following your procedure. If you were sedated, you will be comfortable and drowsy when you leave the office. Most patients prefer to go home and rest with no other physical or scholastic activities planned for a few days. With any medical procedure, there can be unexpected results. These can include delayed healing, infection and post-operative numbness or tingling in your lip, chin, or tongue. The oral surgeon will review relevant post-operative events with you and answer any questions during your office visit.

Damage to Sensory Nerve:

A primary concern is a nerve within the lower jaw bone that supplies feeling to the lower lip, chin, and tongue. This nerve is frequently very close to the roots of the lower wisdom teeth. Having these teeth out between the ages of 12 and 18 usually provides shorter roots so that the nerve is not so close to the roots of these teeth. Occasionally, when the teeth are removed, and especially in older patients, the nerve can become injured. When local anesthesia wears off, you may experience a tingling or numbing sensation in the lower lip, chin, or tongue. Should this occur, it is usually temporary and will resolve gradually over a period of weeks or months. On rare occasions it can result in a permanent alteration of sensation similar to having local anesthesia. We feel that you should be aware of this possibility before consenting to surgery.

Sinus Communication:

The upper wisdom teeth are situated close to your sinuses, and their removal can result in an opening between your mouth and the sinus. Once again, if the teeth are removed at an early age, the root formation is minimal, and this complication is very unlikely. However, if it does occur, it will usually close spontaneously, but we may give you special instructions to follow, such as avoid blowing your nose for two or three days following the surgery. You can wipe your nose, but don’t blow your nose. If you have to sneeze, you should sneeze with an open mouth into a tissue. Pressure should not be created in the sinus area, which may dislodge the healing blood clot. If you sense this condition occurring after the surgery, please contact the office. An additional procedure may RARELY be necessary to close the opening.

Dry Sockets:

Dry sockets continue to be the most common problem people experience following dental surgery. They arise due to premature loss of a blood clot in the empty tooth socket. This seems to occur with greater frequency in people who smoke or are taking birth control pills. While both jaws can be affected, they usually occur in the lower jaw on the third to fifth day. They cause a deep, dull, continuous aching on the affected side(s). Patients may first notice the pain starting in the ear radiating down towards the chin.

The symptoms frequently begin in the middle of the night, and your pain medication regimen may not help. Treatment can involve changing your prescription. Occasionally it is helpful to place a medicated dressing in the empty tooth socket. This will help decrease the pain and protect the socket from food particles. The effectiveness in alleviating the pain lasts for 24-48 hours and may require dressing changes every day or two, for five to seven days. Dressings usually are removed when you have been pain free for 2 to 3 days.

The dressing doesn’t aid in healing. The only reason to place a dressing is for pain control. If medication is controlling the pain, the socket will heal without a dressing. Following removal of the dressing, an irrigation device may be provided to help you to keep food particles from lodging in the extraction site.

Infection:

Occasionally, post-operative infections occur. This usually requires an office visit and clinical examination. Many times, just placing you on an antibiotic for one week will take care of the infection. If it persists, the area will have to be drained and cleaned. Other temporary problems you may experience in the post-operative period include stiffness of the jaws, chafing around the corners of your lips, facial bruising, and blood oozing from the extraction sites. I needed 4 extractions and I was terrified (I hadn’t been to the dentist in YEARS, due to fear, so my teeth were in bad shape). I chose to be sedated because I didn’t want to be awake and have anxiety. During the consultation, Dr. Butura was friendly and addressed any concerns I had. I also received prescriptions that day which gave me time to get them filled before surgery. He made me feel at ease. His staff members are very nice as well. The day of surgery, I was called in a few minutes after arrival. I was offered a blanket because it was cold, which was a very nice gesture. Dr. Butura entered the room and explained the procedure again. He placed the IV into my arm and it wasn't even a minute later that I was already asleep. I woke up about an hour later, surgery was done. My recovery was great! I went to the movies the same night 🙂 I had very minimal swelling. I followed the prescriptions and only used the ibuprofen for pain. I was so happy that I anticipated a lot of pain but didn’t have it! That’s a pretty good let down to have. I highly recommend Dr. Butura. My experience there was more than what I could have hoped for! Thank you!

I was referred to Dr. Butura for my wisdom teeth removal and it was an outstanding experience. I contacted his office and got an appointment that same afternoon to go over my X Rays, pricing, and insurance coverage. Dr. Butura explained the procedure thoroughly and then scheduled me for the extractions immediately after I signed all the paper work. On the day of my procedure the staff was super patient with me and helped calm my nerves. They constantly made sure I was safe and even helped my dad walk me to the car afterwards. Dr. Butura took excellent care of me and prescribed me with all the needed medications to prevent pain, swelling, and infection. The day after my procedure I even received a courtesy call from one of the receptionists asking if I was doing okay, I could honesty not believe that they took the time to do that and it was a very nice touch. My follow up appointment was today and just like every other time I visited the office, the wait time was minimal and Dr. Butura was direct and to the point. I didn’t have any trouble with my recovery and I was back to work in 2 days, it was great. Lastly, it is important to mention that there were never any hidden fees, I was given an exact amount before scheduling the procedure and that is exactly what I ended up paying, which is unbelievable considering many other places end up having add on fees that aren’t even broken down to explain what they mean. Overall I am so pleased I went with Dr. Butura and would recommend him to anyone looking for a professional and caring oral surgeon.

Confession of a confirmed "antidentite" (a person who hates going to a dentist) In Aug 2019 I was informed that I had a tooth that was split in two from top to bottom. The dentist said it would become infected and needed to be removed. He further said that because of the location the removal would need to be done by an oral surgeon. Now in my mental filing system "oral surgeon" is filed right next to "assassin"......so I was not pleased at this news. When I got to my initial meeting with Dr Butura the people in the office were extremely pleasant, efficient and friendly. (I wondered if they were just trying to prepare me for the ordeal ahead....but no they are really wonderful) Dr Butura explained what the options for treatment were...he was detailed and clear...and answered any and all questions. (Unfortunately he gently, but firmly rejected my option of a couple drops of Gorilla glue and a band around the tooth......I made this in jest, but he said some have actually tried it; unfortunately with severe negative consequences.) There was no rush, as is so often the case. He was patient, clear and reassuring. The day of my feared execution..... When i arrived the staff was very friendly and reassuring that this would be easy. The Bible says: "It is appointed unto man once to die" and I wondered if this was going to be my appointment....... Dr Butura came in and said "lets take a look". He rubbed something around the tooth and walked out. He returned in a few minutes and said: "open wide"....It was then I saw the deadly needle. he spent a few seconds doing something then he said; "OK... I'll be back in 15 minutes". I asked him when he was going to give me the dreaded shot? He replied he just did.....The shots were one of my biggest fears, and he did it with NO PAIN. He is the absolute best at giving shots I have ever met. I was now beginning to think I might actually survive the whole procedure. He came back and once again said: "open wide". He said you will feel a little pressure, but no pain.........A couple minutes later he repeated this......again pressure, but no pain. A minute later he stepped back and said; We're done". I could hardly believe it.....I not only survived....I felt fine.....no pain!!!!! The PA explained in detail what to do for the next several days. On the way out all the staff wished me well. I got a follow-up phone call the next morning. This was the most positive dental experience i have ever had!!! I whole heartedly recommend Dr Butura and his team. PS In my mental filing system I have moved Dr Butura away from "assassin" to two files down from "Guardian Angel".
